Yes I Can! Diabetes Self-Management Program

What it is: The Yes I Can! Diabetes Self-Management Workshop Series is a free six-session workshop that is conducted in two and a half hour sessions in community settings. The program was developed and tested by Stanford University to help people learn the techniques and strategies they need for day-to-day management of their disease. The goal of self-management is to improve quality of life by giving patients the skills to manage their lives in the face of chronic illness. This program does not conflict with existing treatment or other education programs.

Who should participate: Yes I Can! is designed to benefit adults diagnosed with Type 2 diabetes or pre-diabetes symptoms. Participants are encouraged to attend the series with a loved one or support person.

What is taught: Workshop topics include healthy eating, sharing/problem solving, action planning, exercise, preventing hypoglycemia, monitoring glucose, stress/depression, medications, preventing complications, communication skills, and sick days.
