Daily Health Assessment
All students, faculty, and staff are required to complete the Daily Health Assessment prior to their arrival on campus each day. For those living on campus, it must be completed before students depart their residence at the start of each day.
Assessment Questions
- Have I been in close contact with anyone who has tested positive for COVID 19?
- I took my temperature today, and it is over 100.4.
- Do I currently have any of these symptoms:
- difficulty breathing
- a new loss of taste or smell
- a new cough
- a new headache
- nausea
- diarrhea
If you answered yes to any of the above, or if YOU have tested positive for COVID-19 in the last two weeks and have not reported that test result to MTSU Student Health, please contact MTSU Student Health immediately at 615.494-7745.
