Upper Division Form

• Two semesters before you intend to graduate, you must complete an upper-division form and file it with your graduation coordinator.
• If you transfer to MTSU with 60 credit hours or more, you need to file this form your first semester at MTSU.
• Your advisor will help you complete this form.
• After your upper-division form is processed, you will receive a letter and degree audit from your graduation coordinator, stating which courses you lack to graduate in your major.
• This form may be obtained from your advisor or the department of your major.

Intent to Graduate Form

• All degree candidates must file a Notice of Intention to Graduate, two semesters prior to graduation.
• This form can be printed out here.

DegreeWorks on PipelineMT

You can view your DegreeWorks on PipelineMT. To do so,

1. Log into PipelineMT.
2. Click on Registration & Student Records.
3. Select DegreeWorks in the Academic Records box.
