Report number and/or name of involved party.
Scanned copy of your government issued photo ID.
If you are unable to scan a copy of your photo ID to attach to this form, you can request a copy of the report in person at the Middle Tennessee State University Police Department, 1412 East Main Street, Murfreesboro, TN 37132 between 8:00 am – 4:30 pm Monday-Friday, or you may mail your request including the information below, along with a copy of your photo ID.

Involved Parties: A copy of your government issued photo ID is required before release of any police report.
Authorized Representatives: An authorization form completed by the involved party along with your photo ID are required before release of any motor vehicle accident report.
